| INSTRUCTIONS
FOR 0N-LINE GIFT REQUEST APPLICATION |
| On Line Processing
is quick, simple and easy. Only one signed document is needed to
process on line. Form 106 "Sellers/Builders Participating Home
Service Agreement". The total Service fee to Family Home Providers,
Inc. is the flat processing fee plus the gift funds requested.
The following are the steps for on-line processing.

- Go To www.familyhomeproviders.org
and click on the On-Line Application for Lenders Only. A page
will come up directing you to either the Builders Request page
or the Resale Request page. Click on one box needed.
- Information area will come up. Fill in all the
information that is required. "*" marks all the areas
that are required. When completed, double click on "SUBMIT"
at the bottom of the application.
- Once you have submitted the information a pop-up
window will appear prompting you to download the "Sellers/Builder
Participating Home Service Fee Agreement" Form 106. The Sellers
or Builders signature must appear on this form and it must be
faxed to Family Home Providers, Inc. prior to wiring of gift funds.
- When your on-line request is processed by Family
Home Providers, Inc., an e-mail notification will be sent to you
at the e-mail address you provide in the application.
- Reservation of gift funds and the Gift Letter
will be returned to you, by fax, within 1 hour or sooner in an
emergency.
- Gift Funds will only be wired after faxed receipt
of Form 106, and a phone call or a faxed note stating that the
loan has been approved and is ready to close at X time and X date.

FOR LENDER
FAXED (PAPER) REQUEST APPLICATION
The following is the process when applying by fax machine (paper).

- Fill out Form 113 "Down Payment Gift Request",
have a filled out and signed Form 106 "Seller/Builders Participating
Home Service Fee Agreement". Have the closing agent wiring
instructions available. Fax all 3 of these items to Family Home
Providers, Inc.
- A reservation of Gift Funds and the Gift Letter
will be processed and returned to your fax number within 1 hour
or soon if needed.
- Funds will not be wired until Family Home Providers,
Inc. has received a phone call or a note, stating that the loan
has been approved and is ready to close at X time on X date.
- Once all the above has been completed, the
funds will be wired. All this can be done in minutes if needed
in an emergency.
